PATIENT INFORMATION

We want to help set you on a clear path toward life without pain. Our specialists will provide you with a personalized plan to reach your health, fitness, and wellness goals. We are passionate about exceptional care, and work directly with you throughout your treatment toward an active and pain-free lifestyle.

Scheduling & Your First Physical Therapy Session

Depending on your condition, a typical physical therapy session will last approximately 45-75 minutes.

  • Insurance coverage and the cost of physical therapy is unique to your plan. Please review our Insurance 101 Guide for more detail on referrals vs prescriptions, and defining copays, coinsurance, and deductibles. Marathon accepts most major insurance providers.
  • Scheduling out a minimum of four weeks of appointments per your plan of care is highly recommended.
  • Wear comfortable clothing that is easy to move around in and allows clinicians to access your point of pain. Sweatpants, gym shoes and other workout attire is preferred.
  • Please complete your new patient forms and bring them with you to your first appointment, along with any medical assistive devices, such as a brace, splint or crutches, and a list of prescribed medicines you may be on. Most forms are linked below, and there will be additional forms to sign at your first visit.
  • If you are unable to print and complete new patient forms prior to your first visit, please arrive 20 minutes before your evaluation time and we will have this paperwork prepared for you to fill out.
  • A Patient Responsibility Form will be presented to you upon arrival and our staff will read through this with you, including outlining your insurance coverage and personal responsibility.
  • Call us if you are running late, we will try our best to accommodate your schedule. Arriving early is a plus.
  • Check in with the front desk when you arrive. Please do not begin any activities until you receive instruction from the staff.
  • Provide valid photo identification (i.e. driver’s license, state ID or passport) in addition to your insurance card when you arrive.

In addition to our therapy services, many of our clinics offer massage therapy and personal training, as well as other fitness & wellness services. Talk to a staff member if you would like to learn more about these services.

Your Condition & Follow-Up Treatment

During your first physical therapy session, your therapist will ask you detailed questions to determine your diagnosis and current condition. These questions will help your therapist create a personalized treatment plan and map out your journey to recovery.

Depending on your diagnosis and condition, follow-up appointments may consist of:

  • Therapeutic exercises
  • Specialized techniques
  • Ultrasound
  • Heat or cold therapy
  • Discussion of your home exercise progress
  • Ongoing education
  • Massage therapy
  • Private Training

What Happens Next?

Your physical therapy treatments take place in an open and welcoming environment. Each session is focused on your progress and safety. Private rooms are available, as needed during the course of treatment. Your therapist may also have a rehab aide work with you during treatment. These individuals are part of the physical therapy team and are trained to work with patients.

Approximately four weeks after your first visit, your therapist will complete a progress summary highlighting your improvement and recommendations for next steps on your treatment.

Free Screen, Evaluation & Consultation

Depending on your needs, Marathon offers three types of initial services

Free Screen

Free Screen is for a person who has pain or discomfort or pain during their day. Examples are back, neck, headaches, shoulder, hip, knee, ankle, etc. This service does not require insurance, only basic information. This is a 30 minute session with a PT who will help you understand your pain and provide a plan of action.

Evaluation

Evaluation is for a person who is injured, has a prescription or referral from a doctor, or is pre or post operation. The evaluation and plan of care are covered by insurance (MPT accepts most major insurances). When scheduling an evaluation we also try to schedule a minimum of four weeks of followup PT visits.

Fitness & Wellness Consultation

Fitness & Wellness Consultation is for a person free of pain, and looking to begin a program that will improve their strength, flexibility, and cardiovascular health. In this 30-minute session a Marathon Fitness & Wellness Coach will discuss goals, interests, and diet and provide a plan of action including training, resources, education, and accountability.
